I picked up a BC-375 and the Master Oscillator VT-4 tube socket has been replaced by a 5-pin socket with a HY-69 tube in it - has anyone seen a mod using this tube? 

It looks like the person who did it did a good job, but I have ordered a correct 4-pin socket to put it back as an original VT-4.

Never seen it nor read of a modification like that.
Being the owner och four BC-375/BC-191 transmitters it makes me happy that your intention is to go back to original.
