I have a small flat chassis with a transformer assy. on it that says "H.V. SUPPLY TYPE 201".

This is a Tektronix part, and makes +2.5KV, -1.6KV, and -1.5KV. Its obviously for running a CRT.

Inside the transformer looking casing is apparently the transformer itself, and V804, 5, 6, as well as filter caps probably.

Those tubes would certainly be the very miniature rectifiers..

The power tube V803 is a 6AQ5 and the oscillator is a 12AU7.  It is about 3x4.5" in size and the XFMR is about 2" square and 3" tall.

I am trying to find out what model scope this is from, as I wish to use it and need to know more about it. Can anyone say what this is from or for?

The alternative is to examine many, many Tek manuals!

It is apparently not from any scope whose manual is on the internet.
The models reviewed did not have the HV supply on a separate chassis but made onto the main chassis with the other parts.

It is extremely close to the supply in the 524AD instrument which uses a 5ABP_ tube. Same voltages.

Also very close to the supply in the 515 and 516, which uses a T55 CRT. +2325, -1675, -1700

Hopefully this tidbit will help someone else who finds this supply. A nice bright 5" mod scope can be made up.
