Since there are some folks in addition to myself who needed to know, I have researched and test, and can announce the following:

UTC S-40 Connection Data
1 -2 = 120VAC input/primary
3-4  = 5 Volt rectifier winding
5-6-7- 6.3 Volt 3 Amps with #6 CT
13-14-15 6.3 Volt 3 amp with #14 CT
8 & 9 525 secondary #10 CT            ) 1,050 @ 250 mA rating
11-12 425 v secondary w/ #10 CT     ) 850 @ 250 mA rating
