Anybody need the insturction sheet of the B&K 510 adaptor?

I'm sorting through an old tube caddy full of tube and such and came across this instruction sheet for the B&K 510 accessory socket panel for the B&K 500 Dyna-quik tube tester.

I can scan it and email it or just post it here, it'll only be three pages. I'm going to toss the original, since it's pretty moldy smelling,unless someone wants it, then I'll just throw it in an envelope for them.
