You may need to use a radial package and extend the leads.  For example:

594-2222-378-90053
Manufacturer Part #:
BFC237890053
Manufacturer:
Vishay / BC Components
Description:
Film Capacitors 5 nF

Check Just Radios for what you need.  I know he (they) have what you need
The only problem is the minimum order is $20 bucks and they mean it.  I ordered about 18 dollars worth and they would not ship.  So if you can make up an order you are in business.

Go to mouser.com and in their online catalog, select passive components. Then select capacitors, then select the type, poly, ceramic, etc.

Here's one that I found, you would have to enter this number in the search box.   PHE450XD5220JD20R06L2

.022 microfarad 3000vDC.

I gave you the information for a cap that meets all your requirements except that it is radial.
