How does one determine positive and negative on this type of capacitor?
Is it the small indentation near the bottom of the case and if so, is it
positive or negative?  See pdf attachment below.

It looks like an oil cap, non-polarized.

If it's polarized one of those leads, most likely, will be attached to the case. Ohm it out. If not DG is probably right.
